Restoring a Voice That Reflects Your Identity
The human voice is one of the strongest expressions of identity. While many people seek a higher voice, others feel that their voice remains unusually high, strained, or inconsistent with their appearance and personality.
At DrVoice Clinic, voice deepening surgery is not simply about lowering the pitch. Our goal is to create a voice that sounds natural, stable, effortless, and fully aligned with the individual’s identity.
Every patient undergoes a comprehensive evaluation of vocal fold anatomy, laryngeal biomechanics, resonance characteristics, professional voice demands, and personal expectations before the most appropriate treatment is selected.Lowering pitch to a deeper, authoritative tone.
Who Can Benefit from Voice Deepening Surgery?
Voice deepening procedures may be considered for:
* Men with persistently high-pitched voices (puberphonia or mutational falsetto)
* Transgender men whose voice remains higher than expected despite hormone therapy
* Patients with excessive vocal fold tension producing an unnaturally high voice
* Individuals seeking a more mature, confident, and authoritative vocal quality
* Selected patients following previous voice surgery
Not every patient requires surgery. In some cases, voice therapy alone—or in combination with surgery—provides the best outcome.
Our Philosophy
There is no universal operation for voice deepening.
Each patient’s anatomy, vocal fold biomechanics, language, professional voice use, singing requirements, and future expectations are unique. For this reason, treatment must always be individualized.
Rather than focusing solely on lowering the fundamental frequency (F0), we aim to optimize vocal fold vibration, resonance, vocal stability, and overall voice aesthetics.
A successful result is not simply a lower voice—it is a voice that sounds authentic.
Two Original Surgical Innovations
Prof. Dr. İsmail Koçak has developed two original surgical techniques for voice deepening. Depending on the underlying anatomy and biomechanics of the larynx, either technique—or occasionally a combination of both—may provide the most appropriate solution.
Diamond Laryngoplasty™
(Window Anterior Commissure Relaxation Laryngoplasty)
Diamond Laryngoplasty™, originally described in the scientific literature as Window Anterior Commissure Relaxation Laryngoplasty, is an original framework procedure developed by Prof. Dr. İsmail Koçak.
First published in 2008, the procedure represents a refined evolution of relaxation laryngoplasty. A precisely designed window is created in the anterior thyroid cartilage, allowing controlled relaxation of the anterior commissure and reducing excessive vocal fold tension while preserving physiological vibration.
The operation is performed under local anaesthesia with real-time voice monitoring, enabling precise adjustment of the vocal outcome during surgery.
Diamond Laryngoplasty is particularly suitable for patients with:
* Puberphonia (mutational falsetto)
* Persistently high-pitched male voice
* Functional high-pitched voice disorders
* Patients seeking a predictable, stable, and long-lasting reduction in vocal pitch

Laser Relaxation Glottoplasty™ (LRxG)
Endoscopic Voice Deepening Surgery
Laser Relaxation Glottoplasty™ (LRxG) is an original endoscopic voice deepening procedure developed by Prof. Dr. İsmail Koçak for patients who require voice lowering without an external neck incision.
Performed entirely through the mouth using a CO₂ laser, LRxG leaves no visible scar. The procedure releases excessive vocal fold tension while preserving the layered microstructure of the vocal fold and its physiological vibration.
Unlike conventional voice deepening procedures that primarily target pitch, LRxG focuses on restoring healthy vocal fold biomechanics. This often produces a voice that is not only deeper, but also richer, more natural, and more stable.
LRxG is particularly indicated for:
* Patients wishing to avoid an external neck scar
* Patients with sulcus vocalis
* Patients with vocal fold stiffness contributing to an abnormally high voice
* Selected revision cases following previous voice surgery
* Patients in whom vocal fold pathology is the primary cause of the elevated pitch
Choosing the Right Procedure
Diamond Laryngoplasty™ and Laser Relaxation Glottoplasty™ (LRxG) are not competing techniques—they complement one another.
The choice depends on detailed assessment of:
* Vocal fold anatomy
* Laryngeal framework
* Vocal biomechanics
* Voice quality
* Professional voice use
* Singing requirements
* Individual expectations
Our objective is always to select the least invasive procedure capable of producing the most natural and durable result.
Recovery
Following surgery, a short period of voice rest allows the tissues to heal properly.
As healing progresses, individualized voice rehabilitation helps patients adapt to their new vocal mechanism and achieve optimal long-term stability.
Although patients usually notice an immediate lowering of their voice, the final result continues to mature over several months as swelling resolves and tissue remodeling is completed.
Expected Results
The success of voice deepening surgery cannot be measured by pitch alone.
At DrVoice Clinic, our goal is to achieve:
* A naturally deeper voice
* Improved vocal confidence
* Greater vocal stability
* Richer resonance
* Preservation of vocal flexibility
* Long-term anatomical stability
* A voice that reflects the patient’s identity rather than simply producing a lower number on a frequency scale
Why DrVoice Clinic?
Voice deepening surgery is one of the most demanding procedures in laryngology.
For more than three decades, Prof. Dr. İsmail Koçak has dedicated his career exclusively to voice disorders and phonosurgery. His work has led to the development of original surgical concepts that have expanded the treatment options available for patients seeking a deeper, more natural voice.
At DrVoice Clinic, treatment is guided not only by anatomy, but also by biomechanics, voice aesthetics, and the unique vocal identity of every individual.
Because every voice is unique, every surgical plan is unique.
